Chronic Care Management
Studies done by Medicare and other sources such as the CDC have shown that close continuous monitoring of persons with two or more chronic conditions reduces stress, improves outcomes, improves patient satisfaction, and improves one’s overall quality of life.
Patients are eligible for CCM services if they have multiple (two or more) chronic conditions. Examples of chronic conditions include, but are not limited to the following: diabetes, high blood pressure, heart disease, depression and osteoporosis.

The Centers for Medicare & Medicaid Services (CMS) recognizes Chronic Care Management (CCM) as a critical component of primary care that contributes to better health and care for individuals
